Family-Centered Care Solutions (“FCCS”) provides RN-led consultation, care coordination, education, and oversight services intended to support clients, families, and residential providers in navigating healthcare needs.

FCCS services are non-emergency and non-diagnostic in nature. FCCS does not provide medical diagnosis, prescribe treatment, or replace care provided by physicians, home health agencies, hospice services, or emergency medical providers.

FCCS services are advisory and supportive and are based on information provided by the client, family members, and/or care teams. Clients remain responsible for all medical decisions and for following the advice and treatment plans of their licensed healthcare providers.
FCCS does not provide emergency services. In the event of a medical emergency, clients should call 911 or seek immediate emergency medical care.

All services are private pay unless otherwise specified in writing. Use of this website or submission of information through contact forms constitutes acknowledgment and acceptance of these terms.
